If you are an independent agent and would like to submit business, you must first be contracted with us. To be considered for a contract, you must furnish us with the following material:
1) A completed Agency Questionnaire. To view the Agency Questionnaire, you will need Adobe Acrobat Reader. |
2) A copy of a valid agent and/or agency license for those states in which submitted business will be located. |
3) Evidence of Errors & Omissions insurance with a minimum policy limit of $1,000,000. |
